Alaska Federal Health Care Access Network
The AFHCAN project will attempt to bring better health care to Alaskans in rural areas, using telemedicine technologies. AFHCAN will mobilize 248 sites, which will include military installations, Alaska Native health facilities, regional hospitals, small village clinics, and state of Alaska public health nursing stations.
Alliance for Rural Community Health
The Alliance for Rural Community Health, which provides policy, collaboration, project management and technology services to Community Health Centers in Northern California, has developed Mendocino SHARE (Securing Health Access and Records Exchange), a project designed to improve health outcomes and reduce costs associated with caring for uninsured and underinsured chronic disease patients accessing multiple healthcare sites in Mendocino County.
CalRHIO
The California Regional Healthcare Information Organization is a collaborative, statewide effort to support the use of information technology and the creation of a secure health information data exchange system.
Colorado Health Information Exchange
The COHIE project involves four major health centers in the development of a technical prototype for statewide exchange. Several local projects statewide will implement health information exchange among a variety of local providers, hospitals and agencies.

Delaware Health Information Network (DHIN)
The goal of DHIN is to help create a statewide health information and electronic data interchange network for public and private use.
Florida Health Information Infrastructure

Hawaii Quality Healthcare Alliance
Hawaii Quality Healthcare Alliance is a consortium of businesses, physicians and government agencies established to set up a statewide network that will give doctors and patients access to medical records via the Internet.
HealthBridge
HealthBridge is a collaborative healthcare network serving the Greater Cincinnati Healthcare Community in Ohio, Kentucky and Indiana.
Indiana Health Information Exchange (IHIE)
Initiated by the Regenstrief Institute, the IHIE was formed to improve health information efforts among 13 institutions representing public health organizations, hospitals, providers, university researchers, state government and economic development groups.
Indianapolis Network for Patient Care (INPC)
INPC is a partnership among the five major Indianapolis hospital systems, Marion County and the Indian State Department of Health.
Inland Northwest Health Services: Spokane, Washington (INHS)
INHS uses HIT, including CPOE, remote access, EMRs and secure and private records.
Kentucky Health Care Infrastructure Authority
The Kentucky Health Care Infrastructure Authority is a joint venture between the University of Kentucky and the University of Louisville. The Infrastructure will, among other things, establish an electronic health network, the Kentucky eHealth Network.
Louisiana Rural Health Information Technology Partnership
This partnership developed a project to implement an electronic medical record system in the emergency departments of 10 critical access hospitals. The grant will also allow electronic information to be shared with other community-based health care providers such as federally qualified health centers and rural health clinics.
Maine Health Information Network Technology (MHINT)
MHINT was formed to deliver a statewide clinical information-sharing network.

Maryland/DC Collaborative for Healthcare IT
A collaborative group of physicians, community hospitals, healthcare systems, MedStar Health, and academic medical centers, including Johns Hopkins Medicine and the University of Maryland Medicine, are establishing a community care data exchange infrastructure for all healthcare members in the region.
Massachusetts eHealth Collaborative

The Mesa County Health Information Network is being designed to allow participating organizations, Rocky Mountain HMO, St. Mary's Hospital, Community Hospital and
Primary Care Partners share clinical data record for Mesa County residents.
Michiana Health Information Network (MHIN)
MHIN is a community health information exchange utility that offers an electronic health record and other clinical information systems that enable physicians and other healthcare providers in the Michiana area to build longitudinal, person-centered medical records.
Michigan Health Information Infrastructure
Michigan Health Information Infrastructure will be instrumental in the implementation of a statewide virtual health library.
Minnesota Community Health Information Collaborative
The Community Health Information Collaborative will develop a shared information network linking hospitals, medical clinics, academic health programs, public health agencies and other appropriate organizations.
The North Carolina Emergency Department Database (NCEDD)
Sponsored by the Centers for Disease Control and Prevention, NCEDD has developed and implemented a system to collect and analyze timely and secure ED data in a centralized database.
Northwestern University, Informatics Section, Department of Emergency Medicine
The Northwestern University Memorial Hospital Informatics Section is developing a number of projects, including a patient tracking system in the emergency department, wireless system and live data feeds.
Oregon Health Information Infrastructure
Oregon Health Care Quality Organizations' has developed the Oregon Health Information Infrastructure project, designed to advance a health information exchange.
Oregon Community Health Information Network
OCHIN, a collaborative project of stakeholders, safety net health care providers and CareOregon, will design and implement a statewide data infrastructure that will offer practice management software to safety net project partners, roll out medical record software for continuity of care for uninsured Oregonians and activate a data warehouse.
Pennsylvania eHealth Consortium
A consortium of 28 health care organizations plan to build the Pennsylvania electronic patient data network that will be tied into a national system so that patients and their doctors can securely access medical records from any part of the country.
Philadelphia Health Information Exchange
The exchange, which includes the Hospital at the University of Pennsylvania, Children's Hospital of Philadelphia, Presbyterian Medical Center and Pennsylvania Hospital, will let hospitals and clinics exchange electronic medical records among different facilities.
Puget Sound Data Exchange
The Veterans Administration and the Department of Defense are pursuing a series of data exchange initiatives. Cycle 1 of the DoD/VA Bidirectional Health Information Exchange, which allows for real-time, bidirectional exchange of select data sets for patients who receive care from both VA and DoD, was implemented between Madigan Army Medical Center in Tacoma, WA, and the VA Puget Sound Healthcare System.
Rhode Island Quality Institute
The Rhode Island Quality Institute established the Rhode Island Health Improvement Initiative, which will, among other things, implement clinical information systems across the entire physician community and interconnect physicians, hospitals, labs, imaging systems, and other providers within the community.
SAFE Health
Secure Architecture For Exchanging Health Information is an initiative established by Fallon Clinic, Fallon Community Health Plan and UMass Memorial Health Care to share clinical information.
Santa Barbara County Care Data Exchange (SBCCDE)
SBCCDE runs as a partnership between public and private health care organizations, including providers, payers, hospitals, clinics, laboratories, pharmacies and the University of California at Santa Barbara, within Santa Barbara County, and CareScience. Whatcom County Health Information Network
A group within Bellingham, Washington, comprising Whatcom Health Information Network, St. Joseph Hospital, Madrona Medical Group, Family Care Network, three specialty practices, and hospital-based and retail pharmacists, are seeking to improve prescription information exchanges between providers and pharmacists.
Wisconsin Health Information Exchange (WHIE)
The nine-county health information exchange collaborative will create a single portal for three existing health information networks: the EMSystem network for emergency care, the state Public Health Information Network and the state Immunization Registry.
